To receive discounts, member must be current on membership payments and purchase must be made through the app. Exclusions apply (see below). Membership is non-transferrable. Member can cancel membership after making 3 monthly payments. Auto pay is made with payment method you choose in the app and will continue unless notice is given to the clinic via email to SmokyMtnHydration@gmail.com or call to 865-280-0208 at least 10 days prior to next payment date. Clinic will send written notice of cancellation to patient once cancelled.
